Overview:

We are seeking a dedicated and knowledgeable Diabetes Nurse Specialist to join our healthcare team in Gibraltar. The ideal candidate will provide expert care and support to patients with diabetes, helping them manage their condition and improve their quality of life. This role involves working closely with patients, families, and other healthcare professionals to deliver comprehensive diabetes care and education.

Key Responsibilities:

  • Patient Care and Management:

    • Assess, plan, implement, and evaluate individualized diabetes care plans.
    • Monitor and manage patients' blood glucose levels and overall health status.
    • Administer and adjust diabetes medications, including insulin therapy.
    • Provide advanced wound care and management for diabetic foot ulcers.
  • Education and Support:

    • Educate patients and their families about diabetes management, including nutrition, exercise, and medication adherence.
    • Conduct one-on-one and group education sessions on diabetes self-management.
    • Develop and distribute educational materials to patients and the community.
  • Clinical Expertise:

    • Serve as a clinical resource and consultant for other healthcare professionals.
    • Stay current with the latest diabetes research, treatments, and best practices.
    • Participate in multidisciplinary team meetings to coordinate comprehensive patient care.
  • Program Development:

    • Develop and implement diabetes care programs and initiatives.
    • Evaluate the effectiveness of diabetes programs and make necessary adjustments.
    • Advocate for improved diabetes care practices and policies within the healthcare system.
  • Community Outreach:

    • Participate in community health events and screenings.
    • Collaborate with local organizations to promote diabetes awareness and education.
    • Provide support and resources to community members living with diabetes.

Qualifications:

  • Education and Certification:

    • Bachelor’s Degree in Nursing (BSN) required; Master’s Degree in Nursing (MSN) or related field preferred.
    • Current RN license in Gibraltar or eligibility to obtain one.
    • Certification as a Diabetes Nurse Educator (CDE) or equivalent is highly desirable.
  • Experience:

    • Minimum of 3-5 years of clinical nursing experience, with at least 2 years specializing in diabetes care.
    • Experience in patient education and chronic disease management.
  • Skills and Abilities:

    • Strong clinical assessment and critical thinking skills.
    • Excellent communication and interpersonal skills.
    • Ability to work independently and as part of a multidisciplinary team.
    • Proficient in using electronic health records and other healthcare technologies.
